Job Title: Health Educator (CHW/TPEP)

Salary: $23.09 - $29.20 per hour
General Position Summary
This position provides health education in the community, focusing primarily on tobacco prevention and cessation, and social determinants of health. Works to achieve deliverables in the workplan and supports the overall team success. This position also functions as a community health worker to assist clients in the nurse home visiting program, clinic, WIC, or other KCPH programs to connect them with local health and social services.
The health educator engages extensively with the community to plan, lead, and facilitate appropriate interventions and programs in response to the community health assessment and improvement plan. Uses educational materials to inform community members and decision makers on health-related issues and policy interventions. This position includes significant community engagement.
Essential Functions
The following may not be all inclusive and other duties may be assigned.
Help collect and monitor data on significant health issues of Klamath County citizens.
Deliver community health education and prevention programs. Presentations will be for all ages from elementary to older adults.
Coordinate and prepare culturally appropriate community health promotion and prevention information to promote increased citizen health outcomes.
Participate in community outreach to increase awareness of and access to public health and human services. Assist clients in these navigating services.
Deliver in person and online tobacco prevention and cessation classes after becoming a certified facilitator. (KCPH will support and pay for attendance of training)
Become a trained application assister for Oregon Health Plan and assist clients with enrollment (KCPH will support and pay for attendance of training)
Work as a part of a team to achieve both departmental and client-specific goals.
Assist in developing and coordinating health assessments to identify preventable negative health outcomes and identifying potential interventions.
Engage with local, state, and regional community-based organizations.
Plan and participate in local, regional, and state public health promotional and training opportunities.
Work with coordinated care organization to strengthen referrals through the Healthy Klamath Connect platform
Obtain Community Health Worker certification within 1 year of hire (KCPH will support and pay for attendance of training)
Attend community outreach events throughout the county, which may occur in evenings or on weekends
Supervisory Responsibilities
This position has no supervisory responsibilities.

Education, Certification, & Experience
High school degree or GED plus to two years related experience and/or training.
Experience working with diverse populations and providing culturally and linguistically appropriate information preferred.
Spanish bilingual preferred but not required.

Language Skills
Ability to read, analyze, and interpret common scientific journals, simple financial reports, and legal documents such as Klamath County Ordinances. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors.
Mathematical Skills
Ability to work simple math and statistics. Ability to understand descriptive and analytical data.
Reasoning Ability
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
Computer Skills
To perform this job successfully, an individual should have knowledge of Explorer, Google Chrome Internet software; Word Processing software; Excel Database software and PowerPoint software.

Work Schedule
The normal work schedule is 8.00 hours per day 5 days per week; exact schedules are assigned by management. This position may require evening or weekend work on occasion.
It is the attendance standard of the employer that all employees be present and on time each shift they are scheduled to work. Full or partial absence during any scheduled shift, arriving late, or leaving early, for whatever reason, may result in disciplinary action up to and including termination of employment.
Time off may be approved if allowed by applicable policies covering illness, injury, or other personal emergency.
Availability for public health emergencies. "The Public Health Employee may be required to stay at or return to work during public health incidents and/or emergencies to perform duties specific to his/her position, or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and/or emergency." Union employee's working a non-traditional work schedule will be compensated according to contract.
